Physical forms were distributed to grades K and 5. Completed physical forms are
needed in grades 1 and 6. Examinations performed after 08/31/10 can and will be
accepted for students in grades 1 and 6 (physicals) and grades 1, 3 & 7 (dental
exams). These forms are due in the school office by March 2012 .
If you do not have a family physician/pediatrician or dentist, the school physician and
dentist will be available only once in the upcoming school year to perform these exams
on your child. The examinations for these grades are a Pennsylvania requirement.
